Financial Information

Fees can vary significantly, dependent upon the procedure. In general, endodontic treatment is much less expensive than tooth extraction requiring dental implants or a dental bridge. We file insurance as a service to our patients but we do require payment at the time of treatment. For your convenience, we accept Visa, Mastercard, American Express and Discover. We also accept personal checks and cash. Debit and health flex cards can be used with some restrictions. In addition, we partner with CareCredit®, a leader in patient financing, to provide no-interest payment plans. CareCredit® requires a simple application prior to your dental visit (www.carecredit.com) or contacting this office for assistance.

We understand that endodontic treatment is an investment in both time and resources and you will have the opportunity to discuss these variables when scheduling your appointment.

We are committed to helping you maximize your dental benefits and are happy to assist in filing any insurance claims. Dental Insurance is a contract between the insurance company and the policy holder and not a guarantee of benefits. As a courtesy, we will file your claim, however, payment for treatment is due in full at the time of service. Any benefits covered by your insurance provider will then be directed back to the policy holder. We recommend contacting your provider for a detailed list of covered services.  You will be reimbursed according to your plan benefits, usually within 2 – 4 weeks. Note that there is a wide variance in insurance coverage due to the negotiated contract between employer and insurance company.

To file your dental claim most efficiently, please provide the following:

  • Current dental insurance card with address, ID and group number.
  • Policyholder/Insured name, birthdate, ID/SSN, address and phone number.

 

We are participating providers with Ameritas, Principal Life, The Standard, Blue Cross Blue Shield of Nebraska, and Delta Dental PPO Plus Premier

COVID-19 UPDATE: Additionally; when an appointment is made, every effort is made to ensure your safety. Due to high demand of PPE and an increase in appointment time reserved for you to allow for extra cleaning methods; we are collecting a reservation fee of $200 when scheduling appointments. The fee is then applied towards the cost of treatment. This fee is non-refundable should the appointment be missed or cancelled without 24 hours notice. Reappointments are allowed as long as they are made with at least 24 hours notice.
